What is an Affidavit of Personal Responsibility?

An affidavit of personal responsibility is a form that is filled out by the examinee and the proctor. The proctor will sit with the examinee for the exam and fill out the affidavit after the exam is completed. Affidavits are state requirements, there are no exceptions. Quest CE upholds all state requirements for valid credits.
You are able to upload the document directly through your Quest CE account. If you choose this option, you will receive an approval email once the affidavit has been checked into our system. You can also scan/email the form to support@questce.com. Lastly, we will also accept the form sent via fax to (414) 375-3449.
Once we receive the affidavit, you will have access to your certificate of completion via your Quest CE account.
